The following pertinent information is required when a Worker's Compensation patient is referred to Kim Foot & Ankle Centers. The patient can bring it to the office or it can be faxed by the attorney and/or doctor directly to our Worker's Compensation Administrator at 562.426.4418. Claims will not be processed unless all documentation is provided.
- DWC1/Claim Form (Body Parts Defined)
- Appointment Letter
- Letter of Representation or 4603 Letter
- All Available Medical Records
- Contested Issue(s) (We base the report on this/these)
- Prescription/Note from the referring physician/PTP
- Insurance company information: include contact name(s) and phone/fax numbers.
